Relevant department leaders and domestic and foreign industry experts to understand and analyze the contents of the combined medical and nursing services and the development status quo.
Relevant leaders of the National Health and Family Planning Commission pointed out in the interpretation of the status quo and development of community medical care combined services that the current model of China's community medical care combined services mainly includes: community health service center and elderly care institutions, community nursing institutions cooperation; Extension of community health services to families and communities; There are three models of integration of community health service and elderly care service, and the typical cases of Beijing, Shanghai, Shandong and Chongqing under these three models are introduced. At the same time, the analysis pointed out that there are still some obstacles in the development of China's community medical care combination, including "more dragons to control the water", high institutional setting standards (medical and old-age care), not to solve the legitimate career problems of doctors, family bed medical insurance is not recognized, the lack of service personnel, insufficient financial security and other problems.

1. Who are you serving?
Through the setting of evaluation and access standards, scientific identification of the service objects included in the combination of medical and nursing services. Shanghai's experience is to establish a unified assessment system for the needs of the elderly and divide care into six levels. After a unified assessment of the needs of the elderly, different care levels are determined, and the medical insurance payment for home medical care is gradually realized. The service targets in Qingdao, Chongqing, Beijing, Qufu and other areas are: people who have been in bed for more than six months due to illness, disability and other reasons, and have a score of less than 60; Community three no people, widows and orphans, five guarantees, empty nesters over 70 years old, disabled people, etc.; Disabled elderly persons within the jurisdiction; Vulnerable groups such as orphans, widows, disabled elderly.
2. Who will provide the service?
The main body providing the combination of medical and nursing services should be the institution with the best resources and the strongest ability to integrate resources. The experience of Qufu area is to develop different service links for vulnerable groups such as disabled people, long-term bedridden people, empty nests, widows and orphans, disabled people, and elderly people with subsistence allowances, and formulate hard indicators for the bidding of the implementation units of each service link, so as to ensure that the service objects can enjoy the same level of service quality in community centers, nursing institutions, day care institutions, and home and other links.
3. How to provide services?
Highlight the linkage of signing services and resources, identify the elderly as the key group of signing, and provide personalized signing services, including family bed service, home care, remote health monitoring, and health management of specific groups and special diseases.
4. What services do you provide?
Medical and nursing institutions should have medical and non-medical, prevention and treatment, play the characteristics of traditional Chinese medicine, care and comfort and other service functions.
5. Who will pay the operating cost?
In addition to the corresponding subsidies of the government and the scope of medical insurance reimbursement, financial issues such as who will bear the other part of the operating cost and what kind of effect will be obtained are the core issues of whether the institutions providing integrated medical and nursing services can operate normally. The experience of Suzhou area is to establish a comprehensive subsidy fund for long-term care insurance for the elderly, encourage the elderly to contribute to the establishment of long-term care insurance funds for the elderly, clarify the service content of long-term medical care and life care for the elderly, and form a joint force shared by the government, individuals and insurance companies.
6. Who will manage it?
Tianjin, Suzhou and other places have issued corresponding policies, made clear provisions on the service model and medical service content of the combination of medical and nursing care, and formulated corresponding rules and regulations to ensure the normal operation of the institutions.
7. Who will provide security?
It is understood that the relevant departments of the National Health and Family Planning Commission will base on their own functions and advantages in the next step, accelerate the standardization of medical and nursing combination capacity building, and focus on strengthening the construction of urgently needed talents such as rehabilitation nursing. At the same time, we will build an international exchange platform, learn from international experience, strengthen communication and cooperation, and actively explore ways to build multi-level talent capacity.
